Metformin carries a risk of lactic acidosis, a rare but serious condition that can result in death. The risk is increased in patients with significant kidney impairment, liver disease, age 65 years or older, excessive alcohol intake, dehydration, contrast imaging exposure, surgery, heart failure, severe infection, or other hypoxic states. If symptoms of lactic acidosis occur, discontinue use and seek immediate medical attention.
Do not take metformin if you have severe renal impairment with eGFR below 30 mL/min/1.73 m², acute or chronic metabolic acidosis including diabetic ketoacidosis, or a known hypersensitivity to metformin.
Inform your provider if you have kidney or liver disease, heart failure, consume alcohol regularly, are scheduled for surgery or contrast imaging, or are pregnant or breastfeeding. Renal function should be assessed before starting therapy and monitored periodically thereafter. Dose adjustments may be required based on kidney function, age, and concomitant medications.
