Your treatment plan will determine how often you need follow-up appointments and lab work. In general, to remain an active patient you must:
Stay up-to-date with your provider follow-up appointments.
3-4 months for controlled substances like testosterone therapy
5-6 months for compounded weight loss
Complete required lab work prior to prescription processing or refill requests.
Maintain good standing with Superior Health & Wellness.
If you don’t meet these requirements, please schedule an appointment with your superior provider, and complete another set of labs. If you don’t know your patient status please reach out to us directly.
If you submit a refill request and an appointment with a provider is necessary to approve request, a $25 processing fee may apply.

Minimum requirements for all patients:
At least 2 provider appointments per year.
At least 2 completed sets of lab work per year.
No monthly or annual membership fees are required.
Controlled substances (such as testosterone or deca):
To request a refill, patients must:
Have met with a Superior provider in the last 3–4 months.
Have submitted labs within the last 3–4 months.
Maintain good patient standing.
Compounded weight loss medications:
To request a refill, patients must:
Have met with a Superior provider in the last 5–6 months.
Have submitted labs within the last 5–6 months.
Maintain good patient standing.
Important Note: Patients who are not in active status cannot process refills or take advantage of Superior’s promotions or special offers.

Refill requests can only be made through our secured HIPAA-compliant refill request form.
Refill Processing Reminder : You will NOT receive a phone call unless additional information is needed; otherwise, we will process your card on file and you will receive communication confirming your refill has been processed.

To ensure there is no lapse in your medication, you should submit a request for a prescription refill when you have about 2-week’s supply left.
Please submit a refill request through our patient refill portal. -
You will receive your approved Superior products and prescriptions within 7-10 business days. However, delivery times may vary due to factors such as pharmacy processing, shipping, product availability, and completion of payment, which are beyond the control of Superior Health and Wellness.

If you have recent lab work from another provider, you may be able to use those results for your consultation. Please submit a copy of your most recent lab results to our office before your appointment.
Our Superior Health Providers will review these results to determine if additional lab work is necessary.
The minimum lab tests we require are:
CBC (Complete Blood Count)
CMP (Comprehensive Metabolic Panel)
Thyroid Panel
Lipid Panel
Hormone Panel (including Testosterone, Estradiol, FSH, LH, and PSA for men, Progesterone for women)
We recommend submitting your lab results at least 3 days before your appointment to allow time for any additional testing if needed.
If you need to schedule lab work, please do so as early as possible to ensure your results are available by your consultation.
